In his first world-title fight, Mthalane fought Nonito Donaire for the IBF title on November 1, 2008. He lost to Donaire via 6th round TKO. However, Mthalane bounced back and won the IBF flyweight title from Julio César Miranda on November 20, 2009, by beating Miranda via a 12 round unanimous decision. He then successfully defended the IBF flyweight title against Zolani Tete on September 1, 2010, winning the fight via 5th round TKO, and again against Johnriel Casimero on March 26, 2011, winning the fight via 5th round TKO.
Mthalane continued his winning streak and successfully defended the IBF flyweight title against many challengers, including Ricardo Núñez, Andrea Sarritzu, and Muhammad Waseem. He faced his biggest challenge in Yuniel Dorticos, but managed to keep his cool and win the title through a unanimous decision victory on July 22, 2021.
